This position is for a full-time Rehabilitation and Extended (REC) Care Associate Medical Director - Internal Medicine Physician/Family Practice Physician/Geriatrician, for the REC Community Living Center (CLC) and Home-Based Primary Care (HBPC) clinic. This position is responsible for direct care to Veterans in the CLC and responsible for clinical oversight of the CLC and HBPC providers.
The CLC focuses on Veterans in advanced states of chronic disease needing 24-hour care. The 225-bed CLC provides comprehensive, interdisciplinary inpatient services for Veterans with complex and chronic, disabling disease.
HBPC focuses on Veterans in advanced stages of chronic disease, particularly those at high risk of recurrent hospitalization or nursing home placement. The HBPC program provides comprehensive, interdisciplinary primary care services in the homes of Veterans with complex and chronic, disabling disease.
The CLC and HBPC program provide palliative care, rehabilitation, disease management and care coordination services.
Palliative Plus program is a funded strategic initiative and component of the current Palliative and Hospice Care (PHC) program. Palliative Plus focuses on creating a level of support beyond typical Palliative Care focusing on hospice-eligible Veterans who decline hospice enrollment but agree to be followed by the team. This program has been shown to decrease utilization of overall healthcare related costs in the context of providing this level of case management.
Home-Based Memory Care is a funded strategic initiative intended to support aging in place and delay institutional placement by addressing the specific gaps in memory care that often result in placement. The concept of "Aging in Place" is a priority of VHACO-GEC.
TBI and SCI/D clinics are both foundational services of VHA. The SCI/D clinic serves as a spoke site to the hub at the Minneapolis VA HCS.
Responsibilities:
o Comprehensive management of the physical, psychological, emotional, social, and spiritual needs of the Veteran.
o The Associate Medical Director is an integral member of the CLC and HBPC teams and will work with an interdisciplinary team of professionals from medicine, nursing, social work, psychology, chaplaincy and other providers to meet the needs of Veterans and families served by these programs.
o Acts as supervisor to CLC Providers.
o Expected to provide clinical oversight and to participate in peer review of other REC providers through periodic chart reviews. Will act as a surrogate for absent REC providers when needed, to include review of CPRS view alerts, and other duties as assigned.
o Regularly attend the provider meeting, departmental meetings and medical staff meetings.
o Typical work hours are Monday to Friday 8-4.30 PM, and a full-time provider is required to work a minimum of 80 hours per pay period.
o Timely documentation (notes and encounter forms) is necessary in all areas in accordance with facility policy. All administrative work should be completed in a timely fashion.
o Physician will work within their privileges at all times and follow the Medical Bylaws.

The incumbent serves as a Medical Support Assistant (MSA) in the Outpatient Clinic Operations section, Department of Veterans Affairs, Tennessee Valley Healthcare System. The incumbent reports to the Supervisory Medical Support Assistant, Outpatient Clinic Operations of both Primary Care and Specialty areas of the service. The incumbent performs duties related to the receipt, intake, and indexing of health administrative information. The incumbent will interface with personal computer (PC) applications as well as Vista, GUI, CPRS, Skype, Email, and VARR.
The MSA is responsible for scheduling appointments, including interpreting and verifying provider orders in accordance with VHA national scheduling guidelines. Employee plans and organizes work general assistance from the supervisor as to meet local and VA - wide time frames to complete analysis requirements, reports and special assignments and only refers to the non-routine and high technical problems to his/her supervisor. The incumbent is responsible for reporting backlogs, difficulties in meeting deadlines, uncooperative physicians, or difficulty in securing information from clinicians or the record. Employee is responsible for maintaining the confidentiality of information obtained to carry out required duties.
Routinely communicates with physicians, case managers, and other allied health professional staff using sound interpersonal skills and is constantly aware of the sensitive information. Otherwise, works independently making decisions based on guidelines, precedents and regulations and performs duties in such a manner that only minimal supervision is required. Performance is judged in terms of overall effectiveness of the operation through broad general objectives, the adequacy and accuracy of the work, timeliness, and conformance to established procedures, policies, and instructions. The VISN Care Coordination, Program Director (Grade IV) is a dynamic and experienced Nurse Leader. Responsibilities include planning, organizing, directing, controlling and evaluating programs and outcomes; daily attention to complex personnel management problems; participation in executive level decision making deliberations; and provision of guidance and direction for the development and implementation of established standards of nursing practice and program oversight.
As a Nurse Leader the incumbent is responsible for facilitating the fulfillment of the VA Heart of Texas Healthcare System's Mission, Vision and Values and the Plan for Provision of Nursing Care.
The incumbent is responsible for planning, organizing, directing, and controlling an integrated program (including inter-facility transfers, the Bed Management System, and Utilization Management), is a key member of the VISN Nursing team, and may have additional duties assigned as appropriate that is based on knowledge, experience and research for processes related to the Care Coordination Program.
This includes:
Trending and analyzing Care Coordination related data and incidents across the VISN
Interpreting VA Directives regarding care coordination
Providing support, education, and promotion of a community of practice to facility Care Coordinators
Development and dissemination of "lessons learned", innovations and best practices related to Care Coordination for all sites across the VISN
Representing Care Coordination Program on numerous VHA and VISN councils and sub-committees
Participating on VISN decision making, i.e., strategic planning, human and fiscal resource management and inter facility transfers that promote the One VISN One strategic initiative
In addition, the VISN Care Coordination, Program Director keeps VISN leadership appraised of issues, incidents, and overall status of compliance with VHA guidance documents. He/she oversees the scheduling of mandated site visits and ensures the field is notified of changes in guidance as they occur. When incidents occur, he/she may be called upon to represent the VISN at national level meetings to review the incidents. Promoting a Culture of Safety and High Reliability to reduce the risk of harm to Veterans that may occur with Care Coordination.
