An exciting opportunity has arisen for an experienced therapist to join the St Helens UCR /Community Frailty Team. This integrated multi-disciplinary and multi-agency team provides specialist rapid assessment and interventions to our local population of St Helens.

Our progressive working environment manages ‘crisis’ on a daily basis, taking referrals from community, care homes and acute colleagues as well as self-referrals. Your clinical skillset will contribute to people being able to remain safely and supported in their own homes wherever possible and preventing unnecessary A&E attendances.

The post holder will work closely with the multi-disciplinary team comprising of Frailty Consultants, Advanced Clinical Practitioners, Occupational therapists, Physiotherapists, Nurses, Social Workers and support workers.Staff may rotate within the frailty therapy services based at Whiston hospital site.

The service provides a 7 day, 8am-8pm, urgent and timely response to referrals. Following this, a rapid assessment and community intervention for up to 72 hours will support adults ‘in crisis’ with health or social care needs at home.

You will be a champion of an integrated community care approach, a strong team player with experience of MDT working and an excellent communicator. Promote and set high standards of professional nursing care and maintain the delivery of quality care within the trust values, policies, protocols and guidelines. Provide professional nursing leadership to MDT within sphere of clinical responsibility. Lead and co-ordinate the service alongside the lead consultant and the other Band 7 nurses within the team. Take a leadership responsibility for a key service need. Continuing responsibility for the assessment of care needs and the development, implementation, and evaluation of programmes of care. Act as a specialist clinical practitioner demonstrating developed clinical skills and knowledge. Support with the development of a service philosophy and models of care to ensure that all policies and procedures are implemented and to participate in the development of nursing practice. Monitor any impact on the patient experience. Ensure that any patient quality and safety issues are addressed in a timely manner. This includes monitoring and evaluating standards of care by the clinical team and providing regular feedback to the specialty team on the standards of care. Implement evidence-based practice, identify, initiate, manage and evaluate changes that lead to improved and high standards of care and in particular any service changes following feedback from patients. Use information to inform Business Plans for the specialist area. Demonstrate awareness of national and local guidelines relating to specialty (e.g. NICE guidelines) and influence care developments in accordance with these.

2. JOB PURPOSE/SUMMARY The post holder will be practising as an independent specialist clinical practitioner providing patient-centred and evidence-based clinical care, within an agreed scope of competence and practice. The jobholder is responsible for the management of a defined caseload. The jobholder carries continuing responsibility for the assessment of care needs, the development, implementation and evaluation of programmes of care and the setting of standards of care. The jobholder will provide specialist nursing care, treatment and advice to a specific patient group and will contribute to the development of the speciality service. The jobholder will ensure cost effective use of resources. The job holder will support the development of interdisciplinary and whole service working. As a role model they will promote the sharing of good practice and delivery of high quality, effective and efficient care. The job holder will provide training and education to staff groups, patients and families/carers. The post holder will work in partnership with the multi-disciplinary team (MDT) to implement and improve the patient pathway and will be expected to develop new skills to further enhance the quality of the service delivered. Where appropriate, the role will include the day to day leadership and management.

The successful applicant will; Provide an expert level of clinical and technical pharmacy services to the cancer directorate for UHS Act as a lead clinical pharmacy practitioner for adult cancer services responsible for the delivery, development, evaluation and co-ordination of clinical pharmacy services in line with the objectives of the division and the UHS pharmacy strategy Act as a lead clinical pharmacy practitioner for all clinical pharmacy aspects of clinical governance within the adult cancer services including clinical risk, clinical audit and education and teaching Where appropriately qualified, work as a non-medical prescriber to deliver a broad service to the cancer directorate. This will include the undertaking of patient facing clinics, providing prescribing support to the service and supporting the out-patient/in -patient cancer units.
