2.1 To deliver care to patients with CNS and Thyroid cancers at St Jamess University Teaching Hospital including: Out-patient clinics Care of in-patients Responsibility for the delivery of radiotherapy Responsibility for the delivery of systemic therapy Responsibility for the prescription and supervision of radionuclide therapies Contribute to specialist MDT meetings 2.2 To link with consultant colleagues in other relevant site specialist teams within the Leeds Cancer Centre and across the Yorkshire Cancer Network. The post holder will be encouraged to facilitate and contribute to the current clinical research programmes in the department. 2.4 To work with colleagues in clinical oncology to deliver acute on call (currently 1 in 26) for clinical oncology. This includes attendance at hand-over, twice daily ward-rounds to review and manage patients admitted acutely and provide out-of-hours cover and advice to the resident on-call team.

Research Governance The post holder will be expected to comply with all Moorfields policies pertaining to research governance. This will include ensuring that all research activities of the post holder are approved by the Trust and an appropriate ethics committee before commencement. This will also include meeting all the Trust research audit, monitoring and training requirements. Research, Teaching & Audit Moorfields is an important research institution and each individual will be expected to contribute to the research and development, teaching and training within the service. As a result the Trust has a policy concerning all intellectual property that may apply to you. The policy can be found in the Medical Staff handbook or copies can be obtained from the HR Department. Professional Development The Trust provides opportunities for CME and CPD. The timetable will be arranged to allow the candidate to take advantage of these opportunities. The opportunity will be available for the post holder to enrol in a part-time medical education degree course. The postholder will be encouraged and financially supported to work towards a medical education higher degree or accreditation programme. There is provision for at least 2 weeks paid study leave with expenses if appropriate each year. This is monitored by the study leave committee. Revalidation requirements All doctors are responsible to maintain a portfolio of their career over the last 5 years in order to satisfy GMC Revalidation criteria. If successful, you will be required to bring along your supporting information portfolio including recent appraisals documents when joining the Trust. Doctors holding joint and/or various fixed term appointments throughout their appraisal year are also required to demonstrate evidence of an exit report and supporting information from the different appointments they hold throughout their appraisal year. This post will help to continue to facilitate the progression of Care of the Elderly services. It is an exciting time to be joining our team we are ambitious to further develop the model of care for frail elderly patients including our services in areas such as Stroke, Frailty, Orthogeriatrics and Dementia - we are keen to support the successful candidate in specialist interest areas. At Weston there are 3 WTE Care of the Elderly and Stroke physicians, and on the Bristol Royal Infirmary and South Bristol Community Hospital sites 8.4 WTE. At Weston there is a well-developed front door Geriatric Emergency Medicine Service lead by an Associate Specialist, with Advanced Physiotherapy, nursing and pharmacy input concentrating on admissions avoidance. The appointment of this position will be part of the 1:11 current consultant on call rota. The main site for this post at present is Weston General Hospital but as we move to full clinical service integration across UHBW it is anticipated that consultants will have some element of cross-site working to be determined through the job planning process. The appointees job plan will involve covering the ID on call rota, ward, clinics and microbiology services across the different sites. The on-call has a 1 in 6 weekend/bank holiday ward round and consults commitment and a requirement to be available for out of hours telephone advice when on call. The rota will include HCID patients when launched later in the year which may require attendance at site out of hours at short notice. There is no involvement in acute medical on call, nor the microbiology on call service

The post holder will participate in providing a 24/7 stroke thrombolysis service and TIA clinics. They will share with colleagues in Stroke medicine the management of all adult stroke/TIA patients (Hyper acute/ acute/ rehabilitation) referred to the Trust. In addition, the post-holder will have the general medical responsibilities for patients on the stroke wards, and is expected to work closely with the Emergency department, Emergency Assessment units and Critical Care to improve the care of Stroke and TIA patients. The post holder will be expected to participate in morning ward rounds, attend daily MDT for discussion of every patients case, and plan communication to all members of staff. The appointee will participate in the provision of a high quality outpatient service, including patient administration. They will be encouraged to explore and set up other stroke related services like neuro-intervention, newer modalities of imaging to detect early ischemic strokes etc. utilising local resources, and to develop the service as required. This post will cover both the hyper acute stroke unit (HASU) on the Luton site and the stroke unit (Shand ward) on the Bedford site. Although this post will primarily be based at Luton & Dunstable. A 10 PA week consists of 8.5 PAs direct clinical care and 1.5 PAs for Supporting Professional Activity (SPA). Job planning will be used to reflect the needs of the department, ensuring that contracted work is appropriately remunerated. An initial job plan review will be undertaken by the Clinical Director after 3 months and annually thereafter, or if the work pattern changes. Any additional PAs contracted by the Trust will be agreed as a separate part of the job plan by mutual consent. The job plans will be a prospective agreement that sets out a consultants duties, responsibilities and objectives for the coming year. It will cover all aspects of a consultants professional practice including clinical work, teaching, research, education and managerial responsibilities. It will provide a clear schedule of commitments, both internal and external. In addition, it will provide personal objectives, including details of links to wider service objectives, and details of the support required by the consultant to fulfil the job plan and the objectives. The pattern of work may change to meet local changing needs and review meetings will take place in line with nationally agreed guidelines or as superseded by locally negotiated agreements. General Responsibilities as Consultant Physician in Geriatric Medicine The following is not an exhaustive nor exclusive list of the duties of a consultant. The appointee is expected to: Deliver day-to-day clinical care and leadership on the Geriatric Medicine wards or within the Acute Frailty bed base on AMU. Lead the daily ward rounds and board rounds and review new admissions to Geriatric Medicine Develop current Geriatric Medicine practice and patient pathways within West Middlesex Hospital and in conjunction with Service Lead /Clinical Director to further develop the service in line with best Lead and contribute to the development of protocols/clinical guidelines in Geriatric Medicine, audit and other key aspects of the clinical governance agenda. Assume responsibility for the organisation, management and diagnosis of treatment of patients in the hospital, in conjunction with colleagues across the hospital. Please See Full Job Description and Personal Specification. The AMU assesses around approximately 1,000 medical patients per month. There is provision for single-sex accommodation. The AMU comprises of AAU and a Short Stay Ward. AAU includes a dedicated assessment area and Ambulatory Emergency Care. Within the AAU there is 5 monitored trollies where we provide level 2 care. Recently, we have established a Cardiology Clinic within our Ambulatory Care where the Cardiologist provides a review clinic for AMU patients. Short stay wardis currently located in F7 which admits patients whose length of stay is less than 72 hours. There is a regular monthly AMU operational meeting, attended by all members of the multidisciplinary team. The operational policy has been agreed by the Trust. The AMU is working with other members of the trust to increase the number of ambulatory care pathways. This will be supported by the assimilation of community services under the umbrella of the acute trust. The unit works closely with the existing community services, including the Early Intervention Team, rapid response team and Suffolk COPD services to avoid unnecessary admissions wherever possible. The team have also worked with the local practice-based commissioning group to redesign the pathway for patients with suspected DVT, which was launched in early July 2010. The new pathway means that the GP performs the initial clinicalassessment and gives the first dose of anticoagulation, so that the patient has to attend DVT clinic the following day for a booked assessment and ultrasound scan. This is currently under review with an intention to run the service with a specialist nurse.
