The Role

• Conduct needs and intake assessment on severity, key risks and protective factors for intervention (if necessary, Home Visit); provide counselling services to help clients and caregivers to manage issues associated with their disabilities to prepare them for vocational pursuit and/or independent community living
• Conduct assessment of client / family and submit social report with action plans based on assessment of client and family
• Participate actively in case discussions and conferences internally within a multi-disciplinary context and externally
• Work with therapist, workshop supervisors and/or Employment Support staff in planning for client’s individualised care plan
• Network with other agencies to tap on community resources to provide intervention support to clients
• Prepare case history information, psychosocial assessment, social work treatment plans
• Document and maintain case files and prepare reports according to the required documentation standard
• Conduct closure of cases upon achieving sustainable service outcomes for clients’

Community Work

• Identify the changing trends/needs of the employment landscape pertaining to the PWDs and coordinate community resources to enhance their employability
• Network with relevant referral agencies and stakeholders serving PWDs and be part of the outreach efforts

Group Work

• Plan, develop and implement curriculum, psycho-educational training and group work for PWDs and/or their caregivers
• Evaluate effectiveness of the training or group work interventions

Project management

• Assist and/or plan events such as open house or caregivers’ appreciation day or other projects as assigned

Professional Development

• Participate in developmental programs and supervision for personal and professional growth & development.
• Maintain minimum requirements for renewal of membership with professional association

Admin Duties

• Other duties as required/assigned by the supervisor or Head of Division

Job Requirement

• Min Degree in Social Work from recognised institutions.
• Excellent communication and interpersonal skills with demonstrated ability to build relationships.
• Minimum 2 years’ experience in employment or disability work preferred

Job Description

Casework & Case Management

• Establish rapport and build a professional relationships with clients and caregivers.
• Conduct intake and needs assessment during first contact and screening to ascertain the level of complexity, key risks and protective factors of the case and prepare for necessary intervention that includes information and referral to help clients and caregivers.
• Conduct and administer appropriate financial assessment and assistance.
• Provide appropriate intervention and to the families of children attending early intervention.
• Participate actively in case discussions and case conferences within a trans-disciplinary setting.
• Participate actively and contribute home behaviours descriptors with other Early Intervention (EI) professionals to discuss and come up with a holistic family-centred intervention plan under the Early Childhood Holistic Outcomes (ECHO) framework.
• Conduct home and school visits with other early intervention professionals whenever necessary using the Routine Based Conversation as much as possible.
• Prepare case history information, psychosocial assessment and social work intervention plans.
• Plan, organise and lead transition planning briefings and actively follow up with parents and external stakeholders on application to SPED/mainstream schools or transition to the next stage of education.
• Document and maintain case files and prepare reports according to the required documentation standard.
• Conduct referral and closure of cases upon achieving sustainable service outcomes for clients.

Group Work

• Plan, develop and conduct psycho-educational training and group work for children, caregivers and other EI professionals.
• Plan and conduct caregiver and parent support group programmes.
• Plan and conduct orientation programmes for new entrants and transition briefings for graduates.

Collaborations and Outreach

• Network actively with external stakeholders such as pre-schools, special schools, primary schools and community partners.

Professional Development & Education

• Participate in developmental programs and supervision for personal and professional growth & development in accordance with the National Social Work Competency Framework (NSWCF).
• Network with other professionals in the EI and disability sector.

Administrative Duties

• Other duties as required/assigned by the immediate supervisor, Centre Manager or Head of Division.

Job Requirement

• Diploma and above in Social Work or Counselling.
• Relevant experience in the fields of Early Intervention/Special Education or social services is preferred.
• Professionals that are accredited as a social worker/social service practitioner (RSW/RSSP) with SASW are preferred.
• Good communication and interpersonal skills.
• Patient, positive and passionate about working with children with special needs and their families.
• Works effectively both as an individual and as part of a trans-disciplinary team.
