The CEPC is responsible for community capacity development for suicide prevention by serving in a leadership role in communicating with, organizing and managing community partner interactions and in helping to build coalitions to implement suicide prevention programs aimed at reducing Veteran suicide at the community, state, and Veterans Integrated Service Network (VISN) levels. The CEPC serves as a seasoned content matter expert for suicide prevention and public health and facilitates integration of suicide prevention education throughout the community by developing and organizing community partners and coalitions aimed at reducing Veteran suicide at the local, state and VISN level. The CEPC is a subject matter expert (SME) in analysis of current community capacity and organization with a responsibility to help manage the intersection of community and facility resources effectively to support the goal of community based interventions for suicide prevention. The CEPC promotes a public health approach to
suicide prevention.

Oversees outreach to educate community partners, the media, VA and other government stakeholders to enhance the development of suicide prevention coalitions. Identifies the specific needs and interests of each group, determines the strategy for providing information, and tailors
the communications to be effective to each group. Monitors feedback and adjusts the communication and education plans accordingly. Develops and delivers education and outreach services that encompasses underserved veteran populations and expands SP coalition building in communities in the facility's geographical catchment area and may include regional/state events.

This involves assembling and coordinating the activities of a diverse planning team of health professionals and representatives from various services who will assist the coordinator in planning and implementing educational opportunities, well-being activities, and programs for employees. The Employee Whole Health Coordinator has the ultimate responsibility for planning and implementation of the local employee well-being program based on national guidance provided by the national Employee Whole Health program office. The expectation is that the Employee Whole Health Coordinator will address overall well-being issues for employees throughout the medical center and outlying areas including but not limited to nutrition, mental, emotional, social, physical, and environmental well-being based on Whole Health (WH) principles and the WH system of care.

Duties include, but may not be limited to the following:

• Assembling a diverse planning team of interdisciplinary health professionals and representatives from various services within the medical center including labor partners.
• Working with the committees to develop a charter, goals, and strategic plans for the Employee Whole Health program, and ensures the plans are updated annually.
• Conducting needs assessment(s) of employees to determine program needs and interests. Developing core program components based on assessment results.
• Developing and delivering classes, seminars, workshops, etc. on various Employee Whole Health topics including but not limited to nutrition, physical activity, and mental health topics or works to identify and collaborates with relevant subject matter experts.
• Developing training materials for staff educational needs.
• Implementing use of available individual health assessments to determine employee well-being strengths and opportunities (i.e., Personal Health Inventory (PHI), Healthy Living Assessment (HLA)), and tracks employee improvement and goal attainment as appropriate.
• Modeling healthy behavior and provides support and encouragement to employees as it relates to behavior change.
• Promoting the Employee Whole Health program to all employees on all shifts throughout the medical center and Community Based Outpatient Clinics (CBOCs).
• Writing proposals for annual funding opportunities as made available by the national Employee Whole Health program office.
• Maintaining and updating local policies and procedures related to Employee Whole Health.
• Managing program budget and is the fund control point official for the Employee Whole Health program.
• Responsible for the purchase of a variety of products, materials, and goods used in the promotion and administration of Employee Whole Health activities.
• Promoting the model of whole health for Employees, including increasing opportunities to engage in self-care activities
• Educates leaders in well-being culture and behaviors which can support a culture of well-being
• Performs other duties as assigned.

Work Schedule: 7:45am - 4:30pm, 20 hours per week.

The incumbent serves as a Social Science Program Coordinator - Employee Whole Health Coordinator and will have full responsibility for administering all aspects of the local initiative to support the health and well-being of employees. This involves assembling and coordinating the activities of a diverse planning team of health professionals and representatives from various services who will assist the coordinator in planning and implementing educational opportunities, well-being activities, and programs for employees. The Employee Whole Health Coordinator has the ultimate responsibility for planning and implementation of the local employee well-being program based on national guidance provided by the national Employee Whole Health program office. The expectation is that the Employee Whole Health Coordinator will address overall well-being issues for employees throughout the medical center and outlying areas including but not limited to nutrition, mental, emotional, social, physical, and environmental well-being based on Whole Health (WH) principles and the WH system of care.

Duties included but not limited to:

• Contacts eligible participants for enrollment and explain the health coaching process.
• Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones.
• Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings.
• Conducts Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate.
• Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments.
• Actively guides Veterans in the exploration of core elements of personal health planning through individual and group based programs.
• Encourages and assists Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran's goals.
• Develops treatment plans for Veterans focused on behavioral interventions based on current scientific knowledge and research that improve quality of life and reduce risk for chronic disease, or that reverse or slow progression of chronic disease.
• Reviews current health behaviors and develops customized treatment plans with Veterans.
• Coordinates care with Veterans' treatment providers, both primary and specialty care, to ensure.
• Meets with external stakeholders to provide information about the Whole Health Group and individual programs and access to Veterans. are consistent with best practices for treatment and management of chronic illness.
• Performs other related duties as assigned.
• Utilizes motivational interviewing and health coaching techniques and processes, learned through pre requisite health coach training, to assist members in developing a realistic plan to accomplish their goals.
• Fosters an environment of individual responsibility to maximize each Veteran's ability to adhere to a lifestyle management plan.
• Develops trust with Veterans and incorporates trust principles in day-to-day activities. Works collaboratively within a team setting to provide positive outcomes for participants, peers and overall operations.
• Maintains education and knowledge regarding lifestyle (habits and behaviors) and clinical outcomes, including behaviors that improve Veterans' health and well-being and those related to poor clinical outcomes.
• Coordinates with others on the Veteran's health care team to ensure optimal patient care.
• Uses VISTA, CPRS, Microsoft office software, Intranet and Internet and other applicable software to track and support patient progress. Communicate proactively and effectively with Veterans, peers and supervisors.
• Conducts outreach activities to inform Veterans and family members of available programs. identified goals.
• Provides educational opportunities for staff-including lunch and learns, grand rounds, and presentations at staff meetings-in order to promote health coaching and encourage referrals to these services.
• Meets with external stakeholders to provide information about the Whole Health Group and individual programs and access to Veterans.
• Other duties as assigned
